I have been self-employed for many years and am very much aware of the dangers of that freedom.
I also had a terrible habit of moving from reward to reward in form of a cigarette. In my case, I could never smoke on the job site and always had to go outside to get my "reward".
I beat it by still taking my breaks, and now instead of a cigarette, I get a bottle of water or a fruit as a reward.
Now the breaks have become by far more enjoyable and i noticed that my brain is slowly reprogramming itself into needing less and less rewards.
I am not planning to completely stop these short breaks, because i believe that they increase your productivity.
A very nice article in the local newspaper some month ago, showed a picture of man playing a flute. He said that instead of going outside to smoke a cigarette, he now goes outside to play his flute.
