the usual detection limit for cotinine in urine tests is 200 ng/ml. There are tests out there which have a much lower detection limit and are able to detect levels of nicotine intake by passive smoking. I don't think the hospital would use such a sensitive assay because they are not interested in any nicotine taken up by a non-smoker - they actually want to check whether you currently consume nicotine or not.

I think you should be fine for this test and do not need to worry.

If you are unsure try your local pharmacy for a OTC urine nicotine/cotinine test and do it yourself. It's as easy as a pregnancy test ;-).
